Following Jigsaw's grisly demise, Detective Mark Hoffman is commended as a hero, but Agent Strahm is suspicious, and delves into Hoffman's past. Meanwhile, another group of people are put through a series of gruesome tests.

Working in the shadow of an esteemed police veteran, brash Detective Ezekiel “Zeke” Banks and his rookie partner take charge of a grisly investigation into murders that are eerily reminiscent of the city’s gruesome past. Between the events of 'Saw' and 'Saw II', a sick and desperate John Kramer travels to Mexico for a risky and experimental medical procedure in hopes of a miracle cure for his cancer, only to discover the entire operation is a scam to defraud the most vulnerable. Armed with a newfound purpose, the infamous serial killer returns to his work, turning the tables on the con artists in his signature visceral way through devious, deranged, and ingenious traps.

What starts as a poignant medical documentary about Deborah Logan's descent into Alzheimer's disease and her daughter's struggles as caregiver degenerates into a maddening portrayal of dementia at its most frightening, as hair-raising events begin to plague the family and crew and an unspeakable malevolence threatens to tear the very fabric of sanity from them all.
